What to Report

(This list is not all-inclusive)

• Inappropriate alterations to client or financial records
• Suspected forgeries
• Authorizing or receiving payment for hours not worked
• Client rights violations
• Violation of HIPAA and other applicable laws

Ways to Report

• Directly to your immediate supervisor
• Report to any Administration personnel
• Report directly to the Compliance Officer at 607-277-4768 ext 304
• Through a designated toll free number: 1-877-767-1900
• In writing to Anthony Lister, 600 W. Washington Street, Geneva, NY 14456
