Case Management

Adult Single Point of Access (SPOA)

Single Point of Access (SPOA) expedites access to case management, housing and Assertive Community Treatment (ACT) services for adults ages 18+ diagnosed with serious and persistent mental illness who are striving to remain healthy and stable in the community.

Lakeview Mental Health Services coordinates the SPOA committee and the referral process for residents of Ontario and Seneca Counties. Services include intensive and supportive case management; housing through Lakeview’s community residences, licensed apartments and supported housing programs; and linkage to the ACT Team.

Programs can be accessed by completing a SPOA referral form and supplying the required clinical documentation. Based on established eligibility criteria and identified need, the SPOA will make referrals to the most suitable programs. Referral forms may be completed by the individual’s clinician, who can expedite the process by including the necessary clinical documentation. Referral assistance may also be provided though Lakeview’s Outreach Workers: Ann Loughney in Ontario County at (315) 789-0550, or Elizabeth Rietmann in Seneca County at (315) 568-3946. Forms are available through area service providers, on the Lakeview website, or by calling 315-789-0550 to ask for a packet.

Lakeview Adult and Children & Youth Case Management

Lakeview offers Case Management services in Ontario and Seneca Counties. Case managers utilize strength-based and person-centered practices to achieve the goals of illness management and recovery. Case managers coordinate treatment planning, link clients to community resources, provide assistance with daily living, help clients manage crises and provide advocacy when needed.

Lakeview Case Management provides four levels of case management:

  • Intensive Case Management (ICM). This level provides clients with weekly one-to one appointments and 24 hour on call assistance.
  • Supportive Case Management (SCM). This level provides two face to face contacts monthly.
  • Seneca CSS (exclusive to Seneca County).  This level meets with clients a minimum of once every three months.
  • Outreach Services are short-term or urgent involvement and referrals for case management can be made through Outreach to the Single Point of Access (SPOA).
