Forensic Case Management
The Ontario County Forensic Case Management Program is a collaborative effort of Lakeview Mental Health Services and the Ontario County Mental Health Department. Through this program the Forensic Case Manager works directly with persons in the Ontario County Jail who have been identified as having specific mental health needs.
The Forensic Case Manager works with each participant to assess their needs and then to develop and implement a plan to address those needs.
Working with the participant and in conjunction with Ontario County Mental Health Department staff, the Forensic Case Manager will provide advocacy and networking support to link the individual with community based services. These may include:
The Forensic Case Manager will also provide supportive counseling and training as needed, along with assistance in obtaining entitlements and services.
Forensic Case Management is accessed for Ontario County inmates by direct referral while incarcerated.
